    Karakterisasi Paired-Like Homeodomain Transcription Factor 2 (PITX2) sebagai Penanda Molekuler untuk Prediksi Asimetri Mandibula dalam Arah Vertikal (kajian genomik, Lingkungan, dan Radiografi Panoramik)

    Abstract
    Background: The complexity of mandibular asymmetry development in malocclusion explains in part why most treatment approaches for malocclusion are directed to the symptoms rather than to etiology. The understanding of genetic and environmental factors interaction is fundamental to the evidence based practice of orthodontics to determine what the best treatment plan is for each patient in the era of truly personalized orthodontics. Pituitary Homeobox Protein Type-2 (PITX2), an active gene in Nodal Signalling pathway that involves as transcription factor in regulating the symmetrical of vertebrae from embriology to post-natal. Aims: This study was conducted to analyze PITX2 gene as a molecular in predict vertical mandible asymmetry based on genomic, environmental, and panoramic radiograph. Method : This research was an observational analytic with case-control study of 129 outcoming orthodontics patients of Dental hospital, Faculty of Denstistry Universitas Sumatera Utara ranged between 11 – 25 years old. Determination of vertical mandibular symmetry based on Kjellberg’s technique on panoramic radiographs before treatment. Dental record was collected and DNA isolation by buccal swab was taken. The difference between symmetrical and asymmetrical group used Chi-squared test and continue with Odds ratio measurement. DNA material was obtained using buccal swabs, followed by Polymerase Chain Reaction (PCR) and Sanger sequencing with ChromasPro 2.13 software (Technelysium, Queensland, Australia) and then compared to archival data from gene bank number ENSG00000164093 (www.ensembl.org). Result : There was a significant difference in trunk posture symmetry based on vertical mandibular symmetry (p=0.047). The characteristics of PITX2 gene showed three polymophism based on sequensing and showed three polymorphism (rs72554076, rs761511445, rs372257787) in 4’UTR from 16 samples. We found compound mutation and mutation in 46+80 C>A that was never reported in gene bank database which showed condylar height asymmetry. Conclusion : Variability in PITX2 DNA sequence, asymmetry of trunk posture and mandibular asymmetry in vertical directions based on panoramic radiographic should be required in the personal orthodontic treatment approach.
     
    Latar belakang: Kompleksnya perkembangan asimetri mandibula yang dapat menyertai semua maloklusi menjadi alasan perawatan ortodonsia terbatas pada menghilangkan tanda dan gejala dibandingkan faktor penyebabnya. Pemahaman tentang interaksi genetik dan faktor lingkungan merupakan bukti berbasis ilmiah dalam menentukan rencana perawatan terbaik bagi pasien ortodonsia di era perawatan yang benar-benar personal. Paired-like homeodomain transcription factor 2 (PITX2) merupakan salah satu gen aktif dalam Nodal Signalling pathway yang berperan sebagai faktor transkripsi dalam mengatur simetri antara sisi kanan dan kiri dari embriogenesis sampai post-natal. Penelitian ini Tujuan: Penelitian ini dilakukan untuk menganalisis karakterisasi PITX2 sebagai penanda molekuler dalam prediksi asimetri mandibula dalam arah vertikal berdasarkan kajian genomik, lingkungan, dan radiografi panoramik. Metode : Penelitian potong lintang pada subjek yang mencari perawatan ortodonsia ke RSGM FKG USU rentang usia 11-25 tahun. Indeks asimetri mandibula dalam arah vertikal berdasarkan teknik Kjellberg pada radiografi panoramik pra-perawatan. Pengambilan data data intra-oral dari rekam medik dan dilanjutkan dengan DNA isolasi melalui metode swab bukal. Identifikasi perbedaan karakteristik memakai Uji Chi-Square dan dilanjutkan dengan mencari Odds ratio. Kajian Polimorfisme basa nukleotida tunggal (SNPs) dengan metode restriction fragment length polymorphism (RFLP), lalu membandingkan hasil sekuensing berdasarkan data arsip bank gen dengan nomor ENSG00000164093 (www.ensembl.org). Hasil : Ditemukan adanya perbedaan bermakna (p=0.047) gejala simetri postur tubuh berdasarkan simetri mandibula dalam arah vertikal. Terlihat 3 polimorfisme (rs72554076, rs761511445, rs372257787) di 4'-UTR dari 16 subjek serta mutasi gabungan dan mutasi 46+80 C>A yang belum terdaftar di data base Nucleotide Single Polymorphism (dbNSP) dengan karakteristik asimetri mandibula dalam arah vertikal disertai asimetri hubungan dental dan TMD. Kesimpulan Perubahan sekuens urutan basa PITX2, asimetri postur tubuh dan asimetri mandibula dalam arah vertikal berdasarkan kajian radiografi panoramik perlu diperhatikan dalam pendekatan perawatan ortodonti yang personal.
