    Abstract
Background: Preeclampsia (PE) is a pregnancy disorder that occurs in 5-14% of all pregnancies. Serum Hs-C-Reactive Protein (CRP) can be used as a marker for early diagnosis of preeclampsia and can reduce maternal and fetal morbidity and mortality. Maternal uric acid levels are associated with the risk of preeclampsia. Serum uric acid measurement is a sensitive and reliable marker and correlates well with the severity of hypertension in pregnant women with preeclampsia. This study aims to determine the comparison of Hs-CRP and serum uric acid levels in pregnant women diagnosed with preeclampsia with severe features and preeclampsia without severe features. Methods: This study is an observational analytical study with a cross-sectional design in the Department of Obstetrics and Gynecology, H. Adam Malik General Hospital, Medan, RSP. Prof. dr. Chairuddin P. Lubis USU and RSUD. Dr. Pirngadi Medan, February - November 2024. In PE that meets the inclusion and exclusion criteria. Examination of Hs-CRP levels and serum uric acid levels was carried out on the study subjects. Data were analyzed descriptively and were said to be significant if p <0.05. Results: There was a significant difference in Hs-CRP levels in PE with severe symptoms (5.76  2.16 mg/dL) compared to PE without severe symptoms (3.57  0.87 mg/dL) (p=0.004). The mean uric acid levels in PE with severe symptoms (7.17  1.6 mg/dL) and PE without severe symptoms (6.27  1.06 mg/dL) were also significant (p=0.004). Hs-CRP levels did not show a significant correlation with systolic blood pressure in PE with severe symptoms (p=0.560) and PE without severe symptoms (p=0.067). Uric acid levels were also not significant in systolic blood pressure in PE with severe symptoms (p=0.603) PE without severe symptoms (p=0.215). Hs-CRP levels showed a significant correlation with diastolic blood pressure in PE with aggravating symptoms (p=0.004) and in PE without aggravating symptoms it was not significant (p=0.162). Uric acid levels were also significant with diastolic blood pressure in PE with aggravating symptoms (p=0.037) and not significant in PE with aggravating symptoms (p=0.903). The correlation value in PE with aggravating symptoms was 0.469. This indicates that an increase in uric acid levels will be followed by an increase in diastolic blood pressure. There is a moderate correlation (r>0.4 – 0.6) between serum uric acid levels and diastolic blood pressure. Conclusion: The inflammatory response characterized by increased Hs-CRP and uric acid is higher in pregnant women with preeclampsia with severe features.
