Background: The anterior cruciate ligament (ACL) plays an important role in maintaining
knee stability and rupture of this ligament is one of the most common injuries. Various factors are
reported to increase the risk of ACL rupture. Recent studies have suggested that tibial
eminence width and height may also increase the risk of ACL rupture. This study aims to
evaluate the relationship of tibial eminence width and height with ACL rupture.
Methods: This study was an observational analytic study with a cross sectional design.
Inclusion criteria were: individuals aged 18-59 years; diagnosis of ACL rupture based on MRI. All
subjects with a history of pathological abnormalities of the knee were excluded. The width and
height of the tibial eminence were measured by a knee anteroposterior X-ray. Height was measured by
the cephalocaudal length of the tibial eminence, and width was measured by the horizontal length of
the tibial eminence.
Results: This study included 45 subjects, consisting of 25 (55.55%) subjects with ACL rupture and
20 (44.44%) without ACL rupture. The height of medial tibial eminence was smaller in ACL rupture
group than the control group (9.02 ± 1.66 mm vs 10.61 ± 1.09 mm), Eta = 0.411. The lateral tibial
eminence height was higher in the control group than the ACL rupture group (10.16±1.04 mm vs
8.98±1.55 mm), Eta = 0.406. The width of tibial eminence was greater in the control group than the
ACL rupture group (13.70±2.19 mm 9.92±2.76 mm), Eta = 0.607. Conclusion: There is an association
between tibial eminence morphology and the incidence of ACL rupture, particularly shown on the
tibial eminence width, medial tibial eminence width, and lateral tibial eminence height.
Keywords: ACL rupture, eminence height, lateral eminence, medial eminence, tibial
