    Abstract
Background. Multiple cases of sexually transmitted infections (STI) remain to be global public health challenges, especially among adolescents, who have more risks due to biological, social, and behavioral factors. In Indonesia, STI cases are still increasing despite various efforts to raise awareness. SMA Al-Azhar Medan is a private school that combines education with religious values. Objective. to describe the level of knowledge, attitude, and behavior of SMA Al-Azhar Medan students towards STI, to fill the gap in local research. Methods. This research was conducted using cross-sectional descriptive study on 68 from 12th grade students, using simple random sampling. Data were collected through a validated questionnaire with a total of 40 questions assessing students' knowledge, attitude, and behavior towards STIs. Descriptive statistics were used to analyze and the results were presented as frequencies and percentages. Results. The results showed that 47% of the respondents had moderate knowledge about STIs, 26.5% had high knowledge, and the other 26.5% had low knowledge. Regarding attitude, 61.8% showed moderate attitude towards STI prevention and 38.2% had strong attitude. Behavioral variables showed that 57.4% had strong behavior, 33.8% had moderate behavior, and 8.8% had poor behavior. Gender analysis showed that female students showed higher understanding and more positive attitudes about STI awareness and prevention than male students. Conclusion. A large number of students had a moderate understanding of STIs, moderate attitudes, and strong prevention behaviors.
