Background: Breast cancer is the second leading cause of cancer-related deaths among women worldwide. Based on WHO data in 2022, there were 2.3 million women diagnosed and the death toll reached 670 thousand. In Indonesia, according to GLOBOCAN in 2020, the number of new breast cancer cases reached 68 thousand cases and deaths reached more than 22 thousand cases. Triple-negative breast cancer (TNBC) accounts for about 15% to 20% of all new cases. Aim: To determine the incidence rate of TNBC in Medan during the period 2022 – 2023 and its characteristics including age, lymphatic metastasis, tumor lymphocyte infiltration (TILS), and lymphovascular invasion. (LVSI). Method: This study is a descriptive cross-sectional study using a total sample of 109 patients sourced from secondary data of breast cancer patients' medical records at RS Murni Teguh and RS Adam Malik Medan during the period from January 2022 to December 2023. Results: The incidence rate of TNBC in 2022 was 15.4% and in 2023 it was 14.1%, with the majority of the age group being over 50 years old (55%). The histopathological characteristics showed that lymphatic metastasis had the same results, with the majority being grade III at 93%. The most common TILS were patients with low positive TILS (71%), and LVSI was mostly negative (75%). Conclusion: The incidence rate of TNBC in 2022 was 15.4% and in 2023 it was 14.1%. The majority of patient characteristics were in the age group over 50 years old, with most being grade III, the same number in lymphatic metastasis, and most patients had low positive TILS and negative LVSI.
