dc.description.abstractIntroduction. Type 2 diabetes mellitus (T2DM) is a complex metabolic disease with a high prevalence in Indonesia. Insulin therapy is one of the primary interventions to controlblood glucose levels; however, its success is influenced by factors such as the type of insul,in patient adherence, and social support. Objective. This study aims to analyze the profile of insulin usage, blood glucose control, and factors affecting the success of insulin therapy in T2DM outpatients at Prof Dr. Chairuddin P. Lubis Hospital, Universitas Sumatera Utara. Methods. This study employed a retrospective observational design with a descriptive approach. Data were collected from medical records of 396 T2DM patients who met the inclusion criteria, including at least three outpatient visits during 2022- 2023, use of insulin therapy, and complete clinical data. The parameters analyzed included the type of insulin (single or combination,) HbAlc levels, fasting blood glucose (FBG), and factors such as family support and patient perceptions. Results. All patients (100%) used insulin analogs, with a combination of rapid­ acting and long-acting insulin being the most frequently prescribed regimen (77.3%). Blood glucose control remained suboptimal, with 85.6% of patients having HbAlc levels > 7% and 80.8% with FBG levels >130 mgldL. The main barriers to successful therapy were low family support and negative perceptions of insulin among patients. Conclusion. The findings highlight the need for optimized insulin therapy through continuous patient education, enhanced family support, and personalized therapeutic approaches tailored to individual needs. These strategies are expected to improve patient adherence and glycemic control, thereby reducing long-term complications.en_US
