Background: Ischemic stroke is a neurological syndrome caused by vascular injury, resulting in infarction in the central nervous system. Type 2 diabetes mellitus (DM), a risk factor for ischemic stroke, is a progressive and chronic condition characterized by insulin resistance. HbA1c serves as an indicator for diagnosing type 2 DM and is a risk factor for ischemic stroke. Objective: To analyze the relationship between HbA1c levels and the occurrence of ischemic stroke at H. Adam Malik General Hospital, Medan, during the 2022–2024 period. Methods: This was an observational analytical study with a cross-sectional design. This observational analytical study employed a cross-sectional design. Medical records provided a population of 4,767 individuals, from which 200 were selected as the sample. The sample was divided into two groups: 100 individuals with type 2 DM and ischemic stroke and 100 with type 2 DM only. The statistical significance of their relationship was then examined. Results: Statistical analysis was performed using the independent t-test. The results showed a significance value (p-value) of 0.476, indicating no significant relationship (p < 0.05). Conclusion: There is no significant relationship between HbA1c levels and the occurrence of ischemic stroke in type 2 DM patients at H. Adam Malik General Hospital, Medan, during the 2022–2024 period.
