Introduction: Interleukin-6 (IL-6) is an inflammatory mediator that increases after surgical procedures such as craniotomy. Anesthesia in craniotomy considers cerebral blood flow, intracranial pressure, and hemodynamic stability, with propofol inhibiting IL-6 production and reducing intracranial pressure, and thiopental having neuroprotective effects. Both thiopental and propofol provide synergistic benefits in controlling intracranial pressure and protecting the brain, improving patient care, and reducing the risk of complications. Objective: To determine the difference in interleukin-6 levels in maintenance doses of intravenous anesthesia propofol and thiopental in craniotomy patients. Methods: This study was a double-blind experimental study to evaluate the effect of propofol and thiopental maintenance on interleukin-6 levels in craniotomy patients at RSUP Haji Adam Malik Medan and RS TK II Putri Hijau Medan which was carried out for one month. Furthermore, the data were analyzed using SPSS software. Results: This study involved 22 samples where there was no significant difference in sample characteristics and hemodynamics before surgery between groups A and B (p>0.05). Changes in IL-6 values were obtained before and after craniotomy surgery, where group A experienced a change of 1.68 (SD 0.94) and group B 6.46 (SD 5.21), and a significant difference was found between the two groups (p 0.000). Conclusion: The study showed a more significant decrease in IL-6 levels in patients receiving thiopental compared to propofol after craniotomy. The use of a combination of thiopental and propofol can provide synergistic effects. Keywords: Interleukin-6, Craniotomy, Anesthesia maintenance, Propofol, Thiopentalen_US
