Introduction. Neonatal sepsis is a major cause of morbidity and mortality in newborns. Neonatal sepsis is a clinical syndrome of bacteremia diagnosed in infants under 28 days of age, characterized by a variety of systemic infection symptoms, circulatory shock, and organ failure in several body systems. Early recognition and understanding of clinical signs, supported by markers of infection, are key in the diagnosis and management of sepsis. In patients with sepsis, infection marker parameters such as leukocytes, I:T ratio, PCT and CRP will experience typical changes. Objective. This study aims to identify the relationship between infection markers and the incidence of early-onset neonatal sepsis. Methods. The design of this study was an observational analytic with a cross-sectional study conducted on 55 samples at Haji Adam Malik General Hospital and 52 samples at CPL USU Hospital of neonatal sepsis patients suspected of neonatal sepsis obtained from medical records from February 2022 to February 2023. Data analysis used logistic regression tests to determine the significant relationship between infection markers and the incidence of early-onset neonatal sepsis. Results and Discussion. From the group with positive blood cultures, the results of the multivariate logistic regression test of leukocytes and I:T ratio showed an α value <0.05, so it can be concluded that both variables significantly influence the incidence of early-onset neonatal sepsis. Conclusion. There is a significant relationship between leukocytes and I:T ratio on the incidence of early onset sepsis.en_US
