Analisis Pelaksanaan Program Terapi Pencegahan TBC (TPT) untuk Penanganan Infeksi Laten TB Paru di Puskesmas Medan Johor Tahun 2024

Abstract

In Indonesia, there are approximately 1.7 million cases of Latent Tuberculosis Infection (LTBI) that have come into contact with active TB cases and the general population. LTBI cases in Indonesia are treated with Tuberculosis Preventive Therapy (TPT), targeting children under 5 years old who have had close contact with active TB cases, household contacts of all ages, and other high-risk groups. Based on data from Medan Johor Health Center in 2022, there were 407 cases of tuberculosis and 4 cases of latent TB. The purpose of this study was to analyze the implementation of the Tuberculosis Preventive Therapy (TPT) Program for the Management of Latent Pulmonary TB Infection at Medan Johor Health Center. This study used a qualitative method through a descriptive approach. Data collection methods included in-depth interviews, observation, and documentation. The results showed that in terms of human resources, facilities, and funding, they were adequate and sufficient for the implementation of the TPT program. The detection of latent TB cases and the promotion of TPT administration were still suboptimal because many people were reluctant to undergo examinations due to the lack of symptoms and the need for more frequent health education at the health center during patient visits. Ensuring the availability of TPT drugs and the management of latent TB, supervised by the PMO, was already optimal and adequate. Recording and reporting were also optimal. The number of latent TB cases and the increase in public awareness were still suboptimal, as the number of latent TB cases found at Medan Johor Health Center was still low and there was a lack of public awareness to undergo examinations, especially those in close contact with active TB patients. The number of cures and the availability of drugs at the health center were already optimal. Regular recording and reporting had been carried out optimally by Medan Johor Health Center. The conclusion of this study is that the implementation of the TPT program at Medan Johor Health Center was still suboptimal, as the detection of latent TB cases was still low, thus hindering the administration of TPT. The recommendation of this study is to further increase the detection of latent TB cases and to enhance health promotion so that the public is more aware of the need to undergo examinations related to latent TB.
