Background. Drug-induced liver injury (DILI) is an acute or chronic liver injury caused by drugs or herbs that cause dysfunction in the liver, with a mortality rate of about 10-50%. Objective. To determine the risk factors for drug-induced liver injury among people living with HIV (PLHIV) taking ARV drugs at Adam Malik Hospital Medan. Methods. This study used an observational analytic method with a retrospective cross-sectional study approach. Data were obtained through medical records at Adam Malik Hospital Medan in the period January 2022-April 2024 with a total sample of 105. The sampling technique used simple random sampling so that 6 samples were obtained with the incidence of drug-induced liver injury. Data were analyzed with univariate, bivariate and multivariate tests. Results. Of the 105 samples, age, gender, CD4 count, type of ARV, duration of ARV use, and history of other drug consumption did not have a significant association with the incidence of drug-induced liver injury. Based on the bivariate analysis, history of alcohol consumption and HBV and/or HCV co-infection in PLHIV taking ARVs had a significant association with the incidence of drug-induced liver injury with p-values of 0.001 and 0.003, respectively. Based on multivariate analysis, HBV and/or HCV co-infection was the only significant risk factor for drug-induced liver injury with an odds ratio of 7.235. Conclusion. HBV and/or HCV co-infection is a factor that influences the incidence of drug-induced liver injury.en_US
