Hypertension is a chronic disease that can threaten the lives of sufferers and has become a world health problem because its prevalence is increasing in various countries, including Indonesia. The prevalence of hypertension in Indonesia in 2013 showed a decrease of 25.8% and in 2018 it showed an increase to 34.11%. One of the efforts to prevent and control hypertension is through the implementation of Posbindu PTM at FKTP. Posbindu PTM is a national program to tackle NCD problems such as hypertension through a preventive and promotive approach with a five-table system. This research aims to determine the implementation of Posbindu PTM at the Medan Sunggal Health Center based on a systems approach, namely input, process and output. The type of research used is descriptive qualitative research with data collection methods of in-depth interviews and observation. Determining informants used purposive sampling, then 8 informants were obtained. The research results show that the implementation of the Posbindu PTM program at the Medan Sunggal Health Center has been running but is not yet optimal. The availability of funds is insufficient to fulfill the procurement of single-use infrastructure such as sugar sticks and cholesterol sticks as well as payment of cadre honorariums which are often late. Incomplete facilities during the implementation of Posbindu such as scales, consumables for sugar and cholesterol sticks as well as KMS FR PTM books for participants, the implementation system used a 5 table system but did not comply with standard operating procedures for Posbindu PTM, recording and reporting were carried out using the ASIK application however Not all cadres understand how to use the ASIK application. Posbindu's target achievement is still low due to posbindu's schedule colliding with community activities. Suggestions to the community health center and program managers to collaborate with many parties in smoothing the implementation of Posbindu PTM and maximizing the 5 stages of Posbindu PTM services in order to increase program targets, so that the success of the Posbindu PTM Program can be measured.en_US
