Abstract

Adolescence is a dynamic phase of development and experiences many changes and problems in adolescent life. These changes include physical, mental, social and emotional changes. The results of the study obtained documentation regarding optimal nursing care in terms of providing health education carried out at SMP 31 Medan, North Sumatra. So based on the problem, it is necessary to improve nursing care by implementing evidence base nu rsing, implementing comprehensive nursing care with appropriate nursing care standards and carrying out nursing documentation.
