Pengelolaan Pelayanan dan Asuhan Keperawatan pada Klien Lansia dengan CKD on HD Regular yang Memiliki Cognitive Frailty di Poli Geriatri RS Pendidikan Prof. Dr. H. Chairuddin P. Lubis Universitas Sumatera Utara

Abstract

Chronic kidney disease (CKD) is a disease that characterized by a decrease in kidney dysfunction that occurs over a long period of time causing the kidney organs unable to function properly. Hemodialysis is one of the kidney replacement therapies that aims to reduce the occurrence of complications and even death. CKD is one of the diseases that is often found among the elderly. Elderly people with CKD who are on regular hemodialysis have a higher risk of frailty than elderly without CKD. The purpose of this senior practice is to apply the theory and concepts of nursing care, especially gerontic nursing, in managing services and providing nursing care to the elderly with CKD on regular HD who have cognitive frailty. Nursing care was carried out on May 20, 2024 - May 22, 2024 for elderly patients in the Geriatric Poly. Diagnoses obtained from the assessment are sedentary lifestyle, insomnia, anxiety, and risk of injury. The results obtained after nursing care was carried out, the patient responded very well and was enthusiastic about hearing the health education provided. Patients understand how to improve sedentary lifestyles, improve sleep, reduce anxiety, and prevent injuries.
