dc.description.abstract | Background: Diabetes mellitus is a metabolic disorder characterized by elevated blood glucose levels caused by insulin deficiency or resistance. Drug-related problems (DRPs) can lead to unmet therapeutic outcomes. an increased risk of morbidity. prolonged hospital stays and a decline in patients quality of life. Objective: This study aims to identify the occurrence of drug-related problems (DRPs) and determine the categories of DRPs affecting patients with type 2 diabetes mellitus at Prof. Dr. Chairuddin Panusunan Lubis Hospital in 2023. Method: This research is a descriptive retrospective cross-sectional study using a total sampling technique conducted at Prof. Dr. Chairuddin Panusunan Lubis Hospital. Data were obtained from 49 medical records of hospitalized type 2 diabetes mellitus patients in 2023. The occurrence of DRPs was identified based on the classification by Cipolle (2012). Results: The study revealed that out of 49 medical records the majority of patients were female (59.18%). aged 56–65 years (42.85%) with a length of stay of 1–5 days (55.10%). The most commonly used single antidiabetic was Novorapid (45.45%) while the combination was Novorapid and Lantus (73.68%). Antihypertensives single therapy with CCBs (57.14%). combination therapy with CCBs+ARBs (27.77%). Of the 49 patients 17 (34,69%) experienced drug-related problems (DRPs). These included 1 case (3.70%) of excessive dosage and 26 cases (96.29%) of drug interactions with moderate significance. Conclusion: Drug-Related Problems (DRPs) occurred in type 2 diabetes mellitus patients at Prof. dr. Chairuddin Panusunan Lubis Hospital in 2023. | en_US |
