Human Error is an action or negligence by an individual that causes an unwanted result such as a product defect. Eternal Mercy is an electric box panel manufacturing industry. According to the research carried out, the company has a problem with the operator's mistake that caused the malfunction. Operator error occurs because the operator does not run the Standard Operating Procedures (SOPs) established by the company. All this time the operator only works according to experience or habit.This research aims to identify and analyze operator errors. The method used is Failure Mode Effect Analysis, to identify Risk Priority Number (RPN) and Cognitive Reliability and Error Analysis Method (CREAM), used to determine Human Error Probability. Based on the highest RPN value of 252 on peeling paint and the CREAM Method result is obtained from the HEP value of 0.728289, where the value belongs to the high category. Recommendations can be made by increasing the monitoring of the implementation of the SOP, providing company-specific sanctions for operators who violate SOPs, holding regular feedback sessions between management and operators to discuss obstacles encountered and provide constructive solutions, reviewing and reviewing SOP regularly to ensure that existing procedures remain relevant and easy to follow, giving rewards or incentives to operators that consistently follow SOP.
