Drug abuse (NAPZA) is a serious issue that impacts physical, psychological, and social aspects. Despite rehabilitation efforts, relapse rates in Indonesia, including at Sentra Insyaf Medan, remain high. A positive self-concept has been proven to support rehabilitation success, while a negative self-concept increases the risk of relapse. This study aims to identify the relationship between self-concept and relapse tendency in NAPZA users at Sentra Insyaf Medan. The research employed a quantitative method with a descriptive analysis design and a cross-sectional approach. The population consisted of 112 NAPZA users, with a sample of 88 users selected using probability sampling. Data analysis was conducted using Spearman's rank correlation. The results showed that out of 88 NAPZA users, 52 individuals (76.5%) had a positive self-concept, while 36 individuals (23.5%) had a negative self-concept. Regarding relapse tendencies, 3 individuals (3.4%) had a very low tendency, 11 individuals (12.5%) had a low tendency, 42 individuals (47.7%) had a moderate tendency, 28 individuals (31.8%) had a high tendency, and 4 individuals (4.5%) had a very high tendency. Statistical analysis revealed a p-value of 0.000 (p<0.05) and a correlation coefficient (r) of -0.591. The study concluded that there is a significant relationship between self-concept and relapse tendency, with a moderate negative correlation. Self-concept is moderately negatively associated with relapse tendencies among NAPZA users at Sentra Insyaf Medan.en_US
