    Abstract
    Background: Conventional risk factors for atherosclerosis are no longer the only risk factors for coronary artery disease (CAD). Excess iron can increase the atherosclerosis process, by forming reactive oxygen species (ROS) through the help of non-transferrin-bound iron (NTBI) which causes increased oxidation of low-density lipoprotein, endothelial dysfunction, monocyte recruitment, formation of foam cells from macrophages, proliferation and migration of vascular smooth muscle cells. One of the parameters for assessing iron status in the body that has good ability is transferrin saturation. This study was conducted to determine the relationship between transferrin saturation and the severity of coronary artery lesions in patients with non-ST-segment elevation acute myocardial infarction (NSTEMI). Methods: This observational analytical study with a cross-sectional design was conducted from February to September 2024 at Adam Malik Hospital. NSTEMI patients underwent iron profile examinations, including serum iron, total iron binding capacity (TIBC), and transferrin saturation, and then underwent coronary angiography. The severity of coronary artery lesions was assessed using the synergy between percutaneous coronary intervention with taxus and cardiac surgery (SYNTAX) score. Results: A total of 72 subjects were involved in this study. From the results of this study, there was no correlation between serum iron and SYNTAX score (p = 0.891) and transferrin saturation with SYNTAX score (p = 0.545). However, there was a negative correlation between TIBC and SYNTAX score (p = 0.004) with a weak correlation value (r = -0.334). Conclusion: Transferrin saturation was not proven to correlate with the severity of coronary artery lesions however we found that TIBC levels were negatively correlated with the severity of coronary artery lesions.
