Background: Stroke is one of the leading causes of death and neurological
disability. Stroke has a negative impact on the quality of life, social and economic
development of patients and their families. Pharmacoeconomic analysis is
necessary to determine the economic impact of a treatment therapy. The cost
effectiveness analysis method is used to determine and select effective health
programs among various treatment options.
Objective: To determine the antihypertensive therapy model used by ischemic
stroke patients and the most cost-effective antihypertensive therapy model at Prof.
dr. Chairuddin Panusunan Lubis educational hospital Medan.
Method: This study is non-experimental and uses a retrospective cohort study
design. Data were obtained from the medical records of ischemic stroke patients
who were hospitalized at the Prof. dr. Chairuddin Panusunan Lubis Educational
Hospital during the period of January 2023-December 2023.
Results: 38 patients who met the inclusion criteria were obtained. Most of them
were female (55%) and aged ≥55 years (66%). The research results show that the
combination of amlodipine + ramipril is an antihypertensive therapy model with the
lowest Cost Effective Ratio (CER) and Incremental Cost Effective Ratio (ICER),
which are Rp 1,651 and Rp -46,333, respectively.
Conclusion: There are 5 antihypertensive therapy models used by ischemic stroke
patients, namely the combination of amlodipine + ramipril, amlodipine, amlodipine
+ candesartan, amlodipine + bisoprolol + candesartan, and amlodipine +
candesartan + furosemide injection. The most cost-effective antihypertensive
therapy model is amlodipine + ramipril.
