    Abstract
    Malocclusion is a condition of deviation from normal of teeth that affects oral aesthetics and function. Adolescents with severe malocclusion feel less confident to smile. IOTN is related to smile analysis, that helps to create aesthetic and attractive smile. Smile aesthetics plays role in diagnosing and planning orthodontic treatment. This study aims to determine relationship between orthodontic treatment needs based on aesthetic components (IOTN-AC) with smile analysis in students with Class I malocclusion at SMPN 30 Medan. This study is a descriptive analytical study with a cross-sectional design. The sample consisted of 41 students selected using simple random sampling method. Data were obtained through direct examination, photometry, and study model analysis. The variables studied included orthodontic treatment needs (IOTN-AC) and smile analysis components including lip lines, smile arches, and smile symmetry. The results showed the majority of students at a low level of treatment needs or did not need treatment (92.7%). Most students had a consonant smile arch (92.7%), but a proportion of students with low lip lines (41.5%) and asymmetrical smiles (46.3%). The relationship between IOTN-AC with lip lines and smile symmetry weak and not significant (p>significant (p = 0.05). In conclusion, there is a relationship between orthodontic treatment needs based on aesthetic components with smile analysis, especially on the smile symmetry component. This study recommends education and routine examinations to detect orthodontic needs early on, as well as further research to deepen the analysis of factors that influence smile aesthetics.
