M (Regional Drinking Water Company) is a Regional Owned Enterprise that works in the field of providing and distributing clean water to the community. Apart from being responsible for distributing water to customers or consumers, this company must improve service and quality so that consumers always feel comfortable. There are several factors that influence the process of water transmission and distribution to customers at PDAM Tirta Nadi Padang Sidempuan branch. The objectives of this research include identifying problems that occur in water transmission and distribution pipes, identifying whether the water sources used meet customer needs, and identifying whether the size of the transmission and distribution pipes is in accordance with predetermined standards. The research method used was the FMEA (Failure Mode Effect Analysis) method and the 5W+1H method. Factors obtained using the FMEA method on transmission pipes include method factors where the transmission pipe is damaged because the transmission pipe has not been replaced, equipment factors where the transmission pipe has not been updated so that the pipe breaks and leaks, and human factors where there is a lack of workers' role in carrying out inspections and repairs, which has an impact on customers. Factors obtained using the FMEA method on distribution pipes include method factors, namely not replacing the distribution pipes so that the distribution pipes are damaged, equipment factors where the pipe size does not meet standards which causes low water pressure and environmental factors where the geographical conditions of the customer's living environment which is located at high altitudes cause the customer to be unable to reach the PDAM flow. The factors obtained by the FMEA method on the number of water sources are the method factor where the search for new water sources is not carried out resulting in a shortage of water sources, the human factor where the number of customers is increasing so that customer needs are not met and the environmental factor where geographical conditions are difficult when searching for new water sources so that water sources are not yet sufficient.en_US
