    Abstract
The Effect of 5% Andaliman Fruit Extract Cream (Zanthoxylum acanthopodium DC.) on Keloid Lesion Improvement Ira Mendrofa, Imam Budi Putra, Nova Zairina Lubis Department of Dermatology and Venereology, Faculty of Medicine, Universitas Sumatera Utara, Medan, Indonesia ABSTRACT Background:Keloids are an abnormal wound healing process characterized by benign fibroblast proliferation that can result in scar tissue extending beyond the original wound edges. Andaliman (Zanthoxylum acanthopodium DC.) is a plant found in Indonesian forests, particularly in North Sumatra. The andaliman fruit has antiinflammatory and wound healing effects that may be beneficial for keloid therapy. Objective: To determine the effect of 5% andaliman fruit extract cream (Zanthoxylum acanthopodium DC.) on the improvement of keloid lesions. Subjects and Methods: This pre-experimental clinical trial was designed as a pretest-posttest study involving 30 subjects with keloids. Diagnosis was established through anamnesis and clinical examination. Evaluation was done using the Vancouver Scar Scale (VSS) and measuring the lesion area before and after applying the 5% andaliman fruit extract cream at weeks 0, 4, 8, and 12. Side effects were recorded during the study, and satisfaction levels were assessed at the end of the study. Friedman tests and post hoc test were conducted to examine differences before and after treatment, with a p-value <0.05 considered significant. Results: There was a statistically significant decrease in VSS and lesion area in subjects with keloid lesions after 12 weeks of applying the 5% andaliman fruit extract cream. No subjects experienced side effects, and all were satisfied with the application of the cream. Conclusion: The use of 5% andaliman fruit extract (Zanthoxylum acanthopodium DC.) cream can improve keloid lesions without side effects and provides a high level of satisfaction. Keywords: keloid, andaliman, Zanthoxylum acanthopodium DC.
