Background: Currently the care of premature babies in the neonatology unit in terms of increasing body weight and the need for proper nutritional intake is urgently needed. Human milk fortifier (HMF) is the preferred method for optimizing the nutritional composition of human milk. Studies have shown that the proper use of HMF can increse the growth rate of eraly premature infants, prevent extrauterine growth restriction (EUGR) and nutritional deficiencies, and does not increase the risk of feeding intolerance (FI). Objective: Treatment of premature babies in the neonatology unit in terms of increasing body weight and the need for proper nutritional intake is urgently needed, namely by giving Human Milk Fortifier (HMF) added to breast milk. The purpose of this study was to determine the comparative analysis of the effectiveness of breastfeeding and Human Milk Fortifier (HMF) with breast milk in premature infants. Methods: Analytical Study - Retrospective Cohort, sample of all premature babies treated in the Neonatology room of H. Adam Malik General Hospital Medan from November 2020 to April 2023. Samples that met the requirements were divided into 2 groups, breastfeeding and HMF or only breastfeeding. Data were analyzed using the SPSS Ver. 25 program with Repeated Anova, Posthoc, and Mann Whitney statistical tests (95% CI and significance p <0.05). Results: The babies weight seemed to show an increasing trend from the first to the 7th day of monitoring in both groups. Using the Mann Whithey test showed that there was a significant difference in delta (weight gain) between the group of babies who were given breast milk and HMF and the group of babies who were given only breast milk (p=0,009). Conclusion: Pemberian ASI dan HMF lebih efektif dibandingkan ASI saja dalam meningkatkan berat badan bayi prematur selama pemantauan 7 hari di ruang neonatologi.en_US
