    Abstract
    Indonesia is a country with a large population and rapid economic growth. This makes the construction of infrastructure, housing, and commercial facilities a growing need. PT Merapi Cipta Karya is one of the manufacturing industries that produces concrete products. One of the products with the highest demand at PT. Merapi Cipta Karya is paving block. The problem faced by PT Merapi Cipta Karya, namely the variation in the compressive strength of the paving block product from the standard of 35 MPa which resulted in a loss of quality. The purpose of this study was to determine the loss of quality and its causal factors to improve the production process of paving block Concrete with Taguchi's Quality Loss Function and Fault Tree Analysis (FTA). The control map shows that the compressive strength of concrete is within the control limits with variations in the range of 34,402 MPa to 35,659 MPa. Calculation of the process capability index obtained Cp = 0,444 indicates the process has not been able to meet the specified standards. Based on the calculation, the loss of quality is Rp110.318.344,2 per month. Loss of quality of Concrete paving blocks Products is caused by 2 main factors (top event), The basic materials that make up paving block do not meet standards and paving blocks have low tensile strength. Based on the Fault Tree Analysis of both top events, the root cause of the loss of quality from the top event of the basic material of paving blocks not meeting standards is the operator working to meet targets, lack of operator accuracy, the material is exposed to rainwater, cheap material prices, inappropriate working methods, and activities that do not add value, while from the top event the cause of the provision of paving block pressure not meeting standards is the operator working to meet targets, lack of operator accuracy, the material contains dirt, inappropriate composition, the provision of pressure is too low, and the mold is not precise.
