D. Kurnia Steel is a business entity engaged in the furniture industry, producing items such as chairs and tables made from wood and metal. The company's logistics network activities begin with the procurement of raw materials from suppliers, based on consumer demand. The risks encountered in the company’s logistics network activities include substandard raw materials, delays in raw material delivery, failure to meet customer demand, and raw material stock shortages. Based on these issues, this study aims to identify the risks occurring in the logistics network activities of UD. Kurnia Steel using the Supply Chain Operation Reference (SCOR) method to map activities, and the House of Risk Phase I method to determine priority risks based on the highest Aggregate Risk Potential (ARP) values, as well as to determine appropriate mitigation actions for the identified risk sources. Based on the risk identification results, 26 risk events and 15 risk agents were identified. From the HOR I data processing, eight priority risk agents were obtained, representing 78.59% of the total risk events, with the highest ARP value being “lack of employee training” at 2784. The results from HOR I were then used as input to determine mitigation actions in HOR II. From the HOR II analysis, eight proposed mitigation actions were obtained. The mitigation action with the highest Effectiveness to Difficulty (ETD) ratio is “implementing a structured and regular training program (safety, SOP, technical).” This mitigation action is expected to reduce employee errors in production planning, thereby preventing product damage and delivery delays by enhancing understanding of company work standards, workplace safety, and technical skills, ultimately aiming to improve the company’s productivity.en_US
