V. Rosen Jaya is a manufacturing company engaged in the production of wooden door panels using a make-to-order (MTO) production system. In its operational activities, the company faces various risks that have the potential to disrupt the smooth flow of its supply chain, ranging from raw material procurement and production processes to the delivery of finished products to customers. This study aims to identify risk events and risk agents that emerge within the supply chain, as well as to design appropriate and effective mitigation strategies to minimize the resulting impacts. The method used in this research is the House of Risk (HOR) approach, which consists of two phases. In the first phase, the Aggregate Risk Potential (ARP) is calculated to determine the most influential and prioritized risk agents for mitigation. Based on the analysis results, eight risk agents with the highest ARP values were selected as mitigation priorities, including operators not reading specifications correctly (A8), sudden large orders from customers (A14), mismatches between production and selected design or color (A15), and issues related to humidity and excessive product stacking (A13). The second phase focuses on formulating mitigation strategies by calculating the Effectiveness to Difficulty (ETD) ratio to determine which mitigation actions are the most effective and feasible to implement. A total of eight mitigation actions were developed, such as assigning job descriptions based on each worker’s skills, building active communication with customers, rechecking and confirming specifications before production, and hiring backup or freelance workers. These mitigation strategies are expected to reduce existing risk potentials and enhance the efficiency and reliability of the supply chain at CV. Rosen Jaya. The findings of this research can serve as a reference for managerial decision-making regarding sustainable supply chain risk management.en_US
