| dc.description.abstract | Shoes are a clothing necessity for all level of society. Shoes are used as footcloths or foot coverings. The large demand for shoes has made many industrial companies take the initiative to make shoes manufacturing a profitable business. UKM Kotama shoes is one of the Company that operates in the fashion sector, namely shoes. This research was carried out spesifically on loafers. UKM Kotama shoes experienced problems in terms of quality control. The average percentage of defect products on loafers reach 1,84%. This exceed the tolerable limit of UKM Kotama Shoes is up to 0,5% of total production. Types of defect consist of glue stains, poor stitching and skin scheck. The aim of this research is to identify the factors that cause defects and analyse them using the six sigma and FMEA approaches. Based on the six sigma DMAIC analysis, it was found that the average sigma value in the 2024 period was 4,01%. Based on the cause & effect diagram, the factors that cause defects are divided into five categories, namely man, machine, material, method, and environmental factors. Then Failure Mode and Effect Analysis are used to see which factors are the most dominant. Based on FMEA calculations, it was found that the most influential factor resulting defect product of loafers for those three types of defects is the human factor cause of many workers were not careful, this was due to lack of supervision by assistant and because of fatigue with the RPN value of 294. The proposed improvements uses 5W+1H methods as a determination of corrective action plans to implement six sigma quality improvements. | en_US |
