dc.description.abstractIntroduction: Sepsis is a critical health issue with high morbidity and mortality rates. Lactiplantibacillus plantarum Dad-13, an indigenous lactic acid bacterium, has proven immunomodulatory effects and may be a beneficial adjunctive therapy for sepsis by reducing biomarkers. Methods: This randomized, double-blind, placebo-controlled clinical trial examined the effects of L. plantarum Dad-13 probiotics for 4 days in adult sepsis patients in the ICU of RS Adam Malik Medan. Procalcitonin, CRP, and IL-6 levels were measured 1 day before and 4 days after administering 2 probiotic capsules per day. Frequency and consistency of defecation were recorded daily during the observation period. Results: There were no significant changes in PCT (3.39 [0.22–160.4] vs 1.13 [0.16– 128.4]; p=0.061), CRP (0.7 [0.5–14.2] vs 0.5 [0.5–10.8]; p=0.261), and IL-6 (79.47 [8.66–333.23] vs 77.92 [17.97–288.8]; p=0.91). However, significant differences were observed in defecation frequency (H3 Probiotic 86.7% vs Control 33.3%; p=0.03; RR 0.2 [95% CI 0.05–0.76]; H4 Probiotic 93.3% vs 46.7%; p=0.04; RR 0.12 [0.02–0.88]) and fecal consistency (H3 p=0.001; H4 p=0.014) between groups. Conclusion: L. plantarum Dad-13 demonstrates potential as an adjuvant therapy in sepsis by improving gastrointestinal function and showing clinical reductions in inflammatory markers, although statistical significance was not reached.en_US
