The Medan City Government plans to build an underpass at the intersection of Road Ir. H. Juanda and Road Brigjend. Katamso to reduce traffic congestion. Emissions have become a major issue for the global community in recent years due to their significant impact on human health and environmental concerns. The increasing number of vehicles on the road and congestion has made road transportation a major contributor to pollution caused by vehicle exhaust emissions. Therefore, an analysis is needed to determine the impact of infrastructure changes from signalized intersection to underpass on vehicle exhaust emissions. The analysis of the impact of infrastructure changes on vehicle exhaust emissions was carried out through traffic simulation using PTV VISSIM software with two scenarios, namely the do nothing scenario (existing signalized intersection) and the do something scenario (underpass) with the types of emissions analyzed are CO, NOX, and VOC. Based on the results of emission analysis with traffic simulation using PTV VISSIM software, CO emissions decreased by 42.5% from 40.991 grams (do nothing) to 23,567 grams (do something); NOX emissions increased by 23.3% from 7,075 grams (do nothing) to 8.725 grams (do something); and NOX emissions decreased by 37.1% from 9.500 grams (do nothing) to 5.975 grams (do something). This shows that the Medan City Government’s plan to make infrastructure changes have a favorable impact on air pollution levels, with reductions in CO and VOC emissions. However, this infrastructure changes plan still need a further research about impact analysis on vehicle exhaust emissions with various other emission types, various calculations, and various methods as a comparative analysis.
