    Abstract
    Background:. The complexity of therapeutic regimens and the concurrent use of multiple drugs increase the risk of Drug-Related Problems (DRPs), which may impact treatment effectiveness, safety, and patient well-being. Objective: This study aims to explore Tgk. Abdullah Syafi’i Regional Public Hospital, Beureunuen, Pidie District healthcare professional’s perceptions of antidiabetic treatment of T2DM patients and DRPs in T2DM and to identify DRPs. Methods: This research was conducted in two phases. The first phase employed a qualitative approach using in-depth interviews with 22 healthcare professionals in healthcare facilities. Thematic analysis was conducted to explore their perceptions and experiences related to DRPs. The second phase involved a retrospective descriptive analysis of 395 T2DM patient prescriptions to categorize DRPs according to PCNE V9.0. Results: In this study, demonstrated that healthcare professionals held generally favorable perceptions regarding the management of DMT2, with pharmacists exhibiting the most comprehensive understanding of antidiabetic DRPs in comparison compared to doctors and nurses. Based on the prescription analysis, 1,275 DRPs incidents were found which the most frequent DRP category was P.2 (treatment safety) with 1,080 cases (84.71%), Drug interactions were the primary cause of DRPs, predominantly at the moderate severity level (791 cases, 84.51%), followed by minor (141 cases, 14.06%) and major severity level (4 cases, 0.43%). Conclusion: Healthcare professionals’ perceptions underscore the importance of interprofessional collaboration in the prevention and management of DRPs. The active role of pharmacists through pharmaceutical care is essential to enhance treatment safety, patient safety, and the effectiveness of therapy in T2DM patients.
