This study aims to describe the implementation of strategies in realizing the Healthy City Program for public transportation services in Medan City related to the requirements, fulfillment of the need for clean and hygienic public vehicles and free of cigarettes in public transportation, Fulfillment of motor vehicle emission needs, free from interference and routine vehicle inspections. This research method uses a qualitative design with a case study approach found in the field and then developed in depth. The results of the study indicate that the transportation agency does not yet fully have a strategy in realizing the Healthy City program in the orderly traffic facilities and transportation services area, especially in public transportation services in Medan City, such as the unfulfilled fulfillment of clean, hygienic public vehicles and free of cigarettes and motor vehicle emissions.
