| dc.description.abstract | The  management  of  medical  records  is  a  crucial  component  of  the  hospital information  system,  especially  in  the  file  loan  process,  which  supports  the continuity  of  healthcare  services  and  legal  aspects.  This  Final  Project  was conducted  at  the  Archives  Unit  of  Universitas  Sumatera  Utara  Hospital.  The objective  of  this  research  is  to  examine  the  procedure  for  borrowing  medical records  carried  out  at  the  hospital,  specifically  concerning  workflow, documentation, tracking, encountered challenges, as well as staff understanding of the  Standard  Operating  Procedures  (SOP).  The  research  method  used  is  a qualitative  descriptive  method  with  data  collection  techniques  in  the  form  of interviews,  observation,  and  documentation.  The  research  findings  indicate  that: 1)  The  loan  procedure  still  utilizes  a  manual  system  using  a  register  book  and tracer  forms.  2)  The  loan  process  flow  involves  requests  from  relevant  units,  a record search by the staff, documentation, and the return of the documents. 3) The main challenges found are delayed returns, lost files, and the absence of a digital tracking system. 4) Staff strive to follow procedures in accordance with the SOP; however,  regular  training  and  periodic  evaluations  are  not  yet  available.  5) Document  security  is  maintained  through  a  system  of  accountability  and  a requirement  for  notarized  power  of  attorney  letters  from  external  parties. Therefore,  the  medical  record  loan  procedure  at  Universitas  Sumatra  Utara Hospital requires system strengthening and improved human resource capacity to enhance service effectiveness. | en_US |
