    Nilai Indeks Syok Modifikasi, Skor TIMI, dan Skor Grace Sebagai Prediktor Kejadian Kardiovaskular Mayor Selama Masa Rawatan pada Pasien Infark Miokard Akut dengan Elevasi Segmen ST

    Abstract
    Latar Belakang : Skor risiko saat ini pada pasien infark miokard akut dengan elevasi segmen ST (IMAEST) membutuhkan algoritme yang kompleks, seperti skor TIMI dan skor GRACE. Indeks syok modifikasi (HR/MAP) merupakan skor risiko sederhana yang terbukti sebagai prediktor KKvM pada pasien IMAEST. Tujuan dari penelitian ini adalah untuk membandingkan kemampuan prognosis indeks syok modifikasi (ISM), skor TIMI, dan skor GRACE sebagai prediktor KKvM selama masa rawatan di rumah sakit pada pasien IMAEST. Metode : Penelitian ini merupakan penelitian kohort ambispektif terhadap 97 penderita IMAEST yang menjalani perawatan di RSUP HAM sejak Januari 2018 sampai Oktober 2018. Semua subyek penelitian akan dilakukan penghitungan skor ISM, skor TIMI, serta skor GRACE pada saat masuk rumah sakit. Lalu subyek penelitian diikuti selama perawatan di rumah sakit untuk menilai kejadian kardiovaskular mayor (KKvM), yakni mortalitas, gagal jantung akut, syok kardiogenik dan aritmia maligna. Kemudian dinilai kemampuan diskriminasi dengan menghitung nilai AUC dan kalibrasi dengan menggunakan uji Hosmer and Lemeshow untuk masing-masing skor. Hasil : Dari 97 penderita IMAEST dalam studi ini, dijumpai 63 penderita (64.9%) yang mengalami KKvM. Performa diskriminasi ISM dengan nilai AUC 0.727 (IK 95% = 0.627 - 0.827), skor TIMI 0.763 (IK 95% = 0.665 - 0.861), dan skor GRACE memiliki nilai diskriminasi tertinggi yaitu 0.801 (IK 95% = 0.714 - 0.889). Ketiga skor ini memiliki nilai kalibrasi yang akurat dengan nilai kalibrasi ISM R2 = 3.234 (p = 0.863), skor TIMI R2 = 6.807 (p = 0.339), dan skor GRACE R2 = 10.704 (p = 0.219). Kesimpulan : Skor GRACE merupakan model prediksi yang paling baik dibandingkan skor TIMI dan indeks syok modifikasi. Indeks syok modifikasi memiliki keunggulan dalam nilai klinis, dan dapat dijadikan model prediksi alternatif terutama pada daerah yang memiliki fasilitas yang terbatas
     
    Background: Current risk scores of ST-segment elevation myocardial infarction (STEMI) need sophisticated algorithm such as TIMI and GRACE Score. Modified shock index (HR/MAP) is a simple risk score that is proven to be a predictor of MACE in STEMI . The aim of this study was to compare the prognostic ability of the modified shock index (MSI), TIMI, and GRACE score as predictors of MACE during hospital stay in STEMI patients. Method: This cohort ambispective study included 97 consecutive patients with STEMI from January 2018 until Oktober 2018 admitted to Adam Malik General Hospital. MSI, TIMI score, and GRACE scores will be counted to all subjects at the time of admission. Then, subjects were followed during hospital care to assess major cardiovascular events (MACE), which are mortality, acute heart failure, cardiogenic shock and malignant arrhythmias. Discrimination were assesed by using value of area under the receiver-operator curve (AUC), and calibration performance was evaluated using Hosmer and Lemeshow. Result: Of the 97 STEMI patients in this study, 63 (64.9%) patients experienced MACE. Discrimination for MSI was 0.727 (95% CI = 0.627 - 0.827), TIMI score 0.763 (95% CI = 0.665 - 0.861), dan GRACE score had the highest discrimination value which is 0.801 (95% CI = 0.714 - 0.889). And all these three scores have accurate calibration values with calibration for MSI R2 = 3.234 (p = 0.863), TIMI score R2 = 6.807 (p = 0.339), and GRACE score R2 = 10.704 (p = 0.219). Conclusion The GRACE score is the best predictive model compared to the TIMI score and the modified shock index. Modified shock index have advantages in its simplicity with good discrimination and calibration, and can be used as an alternative prediction models, especially in areas that have limited facilities
