Introduction: Liver volume is a critical parameter in planning for liver transplantation and major hepatectomy. Although various standard liver volume (SLV) estimation formulas have been developed globally, none to date are based on the Indonesian population or account for local anthropometric characteristics. This study aims to develop a new predictive formula for liver volume based on a healthy Southeast Asian population, specifically Indonesians. Methods: This observational, multi-center cross-sectional study involved healthy adult subjects from seven teaching hospitals across Indonesia. Anthropometric and demographic data were collected, while actual liver volume (CTLV) was measured using CT-scan image segmentation via 3D Slicer software. Data analysis was performed using multiple linear regression, multicollinearity testing, and correlation analysis via SPSS software version 24.0. Results: A total of 221 subjects from seven different academic hospitals were enrolled in this study. The mean liver volume was 1282.04 ± 367.92 cm³, with a higher average in males than females. The final predictive formula developed was: CTLV = 1.858 × Sex + 14.258 × Height + 28.489 × BMI – 3.102 × Age – 1491.2. The model demonstrated a moderate positive correlation (R = 0.517), with R² = 0.267 and adjusted R² = 0.254. This Formula showed the highest correlation coefficient (r = 0.529) compared to 17 existing international SLV formulas. Conclusion: This Formula demonstrates superior predictive accuracy for liver volume estimation in the Indonesian adult population and holds potential for future clinical application following further validation.en_US
