The presence of Sharia hospitals in Indonesia is a response to the growing need of the Muslim community for healthcare services that align with Islamic values. Currently, Medan has three hospitals that apply Sharia principles. This study aims to analyze the utilization of hospital services based on sharia principles by the people of Medan. This research employed a qualitative design using a phenomenological approach. Data were collected through in-depth interviews, document analysis, and observations. The study was carried out in three hospitals that implement Sharia principles in their services: Haji General Hospital Medan, Malahayati Islamic Hospital, and Mitra Medika Premiere Hospital. A total of six informants participated in this study, three representing the hospitals and three representing patients who received care at these hospitals. The findings indicate that while the people of Medan have utilized hospital services based on sharia principles, their reasons were not primarily due to the sharia-based services. Factors influencing service utilization include trust, knowledge, attitudes, peer recommendation, service quality, prior experience, access, environment, facilities, medical and healthcare personnel, insurance, and healthcare system. Hospitals have implemented sharia principles in their services; however, not all have fully complied with the minimum standards set by DSN-MUI and MUKISI. It is recommended that hospitals applying sharia principles enhance public awareness of the concept of sharia healthcare services and fully comply with the service standards established by DSN-MUI and MUKISI.
