    Abstract
Background: Low knowledge about hypertension and the use of antihypertensive drugs causes non-compliance in hypertensive patients. This non-compliance can cause uncontrolled blood pressure and have a negative impact on the occurrence of other cardiovascular disease complications. Objective: This study aims to determine the level of knowledge and adherence to medication as well as the relationship between age, gender, education level and occupation factors on the level of knowledge and compliance of outpatient hypertension patients at Sibuhuan Hospital, Padang Lawas Regency. Methods: This research is a descriptive study with a cross sectional method using a modified questionnaire on knowledge and adherence to taking medication which was carried out in January-March 2024 at the Sibuhuan Regional General hospital, Padang Lawas district. The analytical tests used are univariate and bivariate analysis with the SPSS Version 22 program. Results: From the results of this research obtained from 110 respondents, it was found that the level of knowledge about hypertension in the high category was 28.18%, medium 50% and low 21.18%, while the level of fulfillment in the high category was 20%, medium 44.54%, low 35.45%. The results of the Chi square test to see the characteristics of respondents with their level of knowledge showed that there was a relationship between knowledge and age level of 0.00 (p<0.05). Meanwhile, there is no relationship with the level of compliance. The results for the correlation test did not show a relationship between the level of knowledge and the level of compliance with a very weak correlation (r=-0.172) with a negative relationship and a significance value (sig=0.072). Conclusion: The level of knowledge about hypertension is in the medium category (50%) and the level of compliance is in the medium category (44.54%). The level of education influences knowledge and age influences the level of compliance.
