    Abstract
    The aging process is a natural phase of life characterized by a decline in the function of various body systems, including the musculoskeletal and nervous systems, which can lead to balance disorders and physical weakness. This condition increases the risk of falls among the elderly, which can result in injuries, reduced independence, and even death. An unsafe environment can also contribute to the risk of falls among the elderly. This study aims to manage nursing care in meeting the safety needs of the elderly in Environment IX, Pangkalan Mahsyur Village, Medan Johor District, Medan City. A case study approach was conducted on three elderly individuals with balance disorders and varying fall risks based on assessments using the Tinetti Balance and Gait instrument. Nursing interventions included balance exercises, fall prevention education, and environmental management to enhance safety for the elderly. Before intervention, two clients, Mrs. B and Mrs. M, were in the high-risk category with scores of 18 and 17, respectively. After undergoing continuous balance therapy, their scores improved to 22 and 20, indicating a reduction to the moderate-risk category. Meanwhile, client Mrs. N, who was previously in the moderate risk category with a score of 20, improved to 24 and moved into the low fall risk category. The results of the management showed improvements in body balance, reduced fall risk levels, and increased awareness among the elderly and their families in creating a safe environment. Thus, it can be concluded that targeted, continuous, and case-based nursing interventions have proven effective in enhancing the safety, independence, and overall quality of life of the elderly.
