    Abstract
    Background: Primary healthcare centers (Puskesmas) play a crucial role in delivering promotive, preventive, curative, and rehabilitative services to the community. Pharmacists are responsible for ensuring the rational use of medicines through pharmaceutical supply management and clinical pharmacy services. The Pharmacist Professional Work Practice (PKPA) provides prospective pharmacists with hands-on experience to strengthen their competence in this area. Objective: This PKPA aimed to enhance the understanding of prospective pharmacists regarding the role, duties, and responsibilities of pharmacists in Puskesmas, and to evaluate the implementation of pharmaceutical care according to national regulations and standards. Method: The practice was conducted at UPT. Puskesmas Bromo Medan from October 1 to 28, 2024, through direct observation, participation, and documentation of pharmaceutical activities, including supply management and clinical pharmacy services. Results: The activities covered planning, requesting, receiving, storage, distribution, controlling, and reporting of pharmaceutical supplies and medical consumables (BMHP). Clinical services performed included prescription review, drug information services, patient counseling, monitoring adverse drug reactions (MESO), and drug therapy monitoring (PTO). The practice provided valuable exposure to real-world challenges and strategies in optimizing pharmaceutical care at the primary healthcare level. Conclusion: The PKPA at UPT. Puskesmas Bromo improved the professional competence of prospective pharmacists in both technical and clinical aspects of pharmaceutical services. It also highlighted the essential role of pharmacists in ensuring patient safety and promoting rational drug use, there by supporting the overall quality of healthcare services at Puskesmas.
