    Abstract
    Self-care is a crucial aspect in the management of type II diabetes mellitus (DM), encompassing dietary regulation, physical activity, adherence to medication therapy, and blood glucose monitoring. Low levels of knowledge and skills in independently managing the disease pose major barriers to achieving optimal blood glucose control. This nursing practicum was conducted individually by the student in Hamlet I, Ujung Labuhan Village, aiming to manage nursing care for a patient with Type II Diabetes Mellitus through a self-care management education approach. The student conducted a comprehensive assessment, situational analysis using the SWOT method, and formulated interventions tailored to the patient's condition and capabilities. The interventions included education on dietary regulation, physical activity, medication adherence, and daily blood glucose monitoring. The intervention was implemented over the course of one week for one assigned patient. Evaluation results showed improved understanding of the importance of self-care management in controlling blood glucose levels, partial adherence to therapy, and positive behavioral changes in the patient, along with increased knowledge in the patient's family as caregivers. A personalized and continuously adjusted educational approach proved effective in enhancing the patient's self-care ability. This practicum demonstrated that interventions based on evidence-based practice and a holistic approach yield tangible results in improving the health status of patients with type II diabetes mellitus.
