Hypertension is a chronic condition that requires long-term medication to maintain stable blood pressure and prevent serious complications such as stroke and heart failure. However, non-adherence to antihypertensive medication remains common, often due to the asymptomatic nature of the disease, leading patients to forget or refuse to take their medication. This study aimed to evaluate the effectiveness of using medication alarms to improve medication adherence among hypertensive patients. The method employed was a case study using the nursing care approach through five stages: assessment, diagnosis, planning, implementation, and evaluation. Three respondents with low adherence levels were selected based on the Morisky Medication Adherence Scale (MMAS-8) and blood pressure measurements. The intervention focused on the use of medication alarms and recording medication intake times on a daily observation sheet. All three respondents showed improved adherence and more stable blood pressure after the intervention. These findings indicate that the use of medication alarms is a simple and effective strategy to enhance adherence among hypertension patients. It is recommended that clients regularly monitor their blood pressure at healthcare facilities, and that health services adopt the use of medication alarms as a supportive strategy to ensure the success of long-term therapy.en_US
