Background: Pharmaceutical services in hospitals are one of the important aspects in efforts to improve the quality of health services, patient safety, and rational use of drugs. Prospective pharmacists need to gain direct experience through the Pharmacist Professional Work Practice (PKPA) in order to be able to understand and carry out the strategic role of pharmacists in hospitals. Objective: The purpose of this Pharmacist Professional Work Practice (PKPA) is to increase the understanding of prospective pharmacists regarding the role and responsibilities of pharmacists in pharmaceutical services and to compare existing practices with the standards set in the Minister of Health of the Republic of Indonesia No. 72 of 2016. Method: PKPA was carried out at H. Adam Malik Hospital Medan in the period March 3–April 30, 2025 with activities including the management of pharmaceutical preparations, medical devices, and consumable medical materials, as well as patient-oriented clinical pharmacy services. Results: Pharmaceutical services at H. Adam Malik Hospital, which include the management of pharmaceutical preparations and clinical pharmacy services, have been in accordance with standards and are running well, although there are challenges in the field in the form of limited number of pharmacists, there is no adequate counseling room, and the monitoring of drug levels in the blood has not been optimal. Conclusion: This Pharmacist Professional Work Practice (PKPA) provides valuable experience in deepening understanding of the importance of the role of hospital pharmacists in ensuring the quality of pharmaceutical services and patient safety as well as honing communication skills in the clinical environment.
