Patient safety now has become a concern in various countries in the country. It is
noted that RSI Malahayati Medan has implemented a patient safety Service system
but in 2024 there were 23 cases of potential incidence of injuries (KPC), 3 cases of
KNC and 7 cases of KTC. This research is a qualitative study with a descriptive
approach aims to know how the patient safety program is implemented by nurses
in the inpatient ward of RSI Malahayati Medan. The research informant is 1 nurse
head and 4 executive nurses in the hospitalization room. The research data was
obtained through interviews and observations, and the data analysis method used
was an interactive analysis model. The results of the research, namely the
implementation of the third goal is increased safety of medicines to be aware of has
been done with separate storage and given a special label. The implementation of
the patient's identification is not yet optimal because there are still nurses who do
not identify patient with two identities. The implementation of communication
between doctors and nurses has not been in accordance with the target safety
standards, because still found nurses who forget to rewrite the orders they received.
The implementation of risk reduction of infection with hand hygiene is not
appropriate to the patient's safety standards, because there are still many nurses
did not maintain hand hygiene at the required times. The implementation of risk
reduction of the patient falls was in accordance with patient safety target standards,
through initial assessment and preventive measures. RSI Malahayati Medan is
expected to improve evaluation and monitoring on the implementation of patient
safety objectives in accordance with the applicable SOP.
