    Abstract
    Introduction. The ring pessary is a conservative management option for pelvic organ prolapse in menopausal women. However, its use may lead to complications such as bacterial vaginosis due to disruption of the vaginal microbiota. Lactobacillus gel has emerged as a potential strategy to restore vaginal microbial balance. This study aims to evaluate the effectiveness of Lactobacillus gel in reducing the incidence of bacterial vaginosis among menopausal women using a ring pessary. Methods. This study employed a pre-post quasi-experimental design without a control group. Eligible menopausal women with a ring pessary were recruited. Nugent scores before and after the administration of Lactobacillus gel were recorded, alongside analysis of demographic characteristics and their association with bacterial vaginosis. Results. There was a significant reduction in Nugent scores post-intervention (p<0.05). The incidence of bacterial vaginosis also showed a significant decline, indicating a beneficial effect of probiotic application. No significant correlation was found between age, marital status, or occupation and the occurrence of bacterial vaginosis. Discussion: These findings support the role of Lactobacillus in restoring healthy vaginal flora, particularly in menopausal women using intravaginal devices. The probiotic exerts its effect through acidification, inhibition of pathogenic bacteria, and biofilm suppression. Conclusion: Lactobacillus gel is effective in reducing bacterial vaginosis in menopausal women using ring pessaries.
