Auditory hallucinations are a common sensory perception disorder in patients with mental disorders, and can trigger aggressive behavior, social isolation, and a decline in psychosocial functioning. Proper nursing interventions are needed to help clients manage their symptoms. The management of nursing care is carried out through Cognitive Behavioral Therapy (CBT) to reduce the frequency and intensity of hallucinations. The intervention was conducted in five sessions at Prof. Dr. M. Ildrem Psychiatric Hospital, Medan, covering initial assessment, cognitive therapy, behavioral therapy, technique evaluation, and relapse prevention. The techniques used include restructuring automatic thoughts, self-control (hardikan), medication education, and increasing social activity. The results show an improvement in the client's ability to recognize, control, and respond to hallucinations adaptively. The client also showed active involvement in developing and implementing relapse prevention strategies, as well as beginning to carry out daily routines independently. The evaluation shows a significant reduction in the intensity of hallucinations and an increase in independence.
