Background: Personality is an important factor in understanding human behavior. One common personality disorder is Narcissistic Personality Disorder (NPD), which is characterized by excessive self-importance, lack of empathy, and a need for admiration. This disorder can interfere with social relationships and potentially cause difficulties in daily life. The Narcissistic Personality Inventory-16 (NPI-16) is an instrument often used to identify NPD, but to date there has been no research on the validity and reliability of the Indonesian version of this measurement tool. Therefore, this study aims to develop a valid and reliable Indonesian version of the NPI-16 that can be used in Indonesia. Method: This diagnostic study used a corrected correlation analysis approach and involved 400 students and staff aged 18 to 60 years at a state university in Medan. The cross-cultural adaptation process was carried out through forward backward translation, expert review, and preliminary testing. Psychometric testing included content validity (expert assessment), construct validity (exploratory factor analysis/EFA), and reliability (Cronbach's Alpha and Intraclass Correlation Coefficient/ICC). Results: The Indonesian version of NPI-16 showed adequate content validity (S-CVI/Ave = 0.98; S-CVI/UA = 0.875). EFA analysis confirmed a one-factor structure (KMO = 0.772; factor loading 0.470–0.740) with a total variance of 50.148%. Internal reliability was assessed using Cronbach's Alpha (Cronbach's Alpha = 0.716) and ICC (ICC = 0.716). Conclusion: This study successfully developed a valid and reliable Indonesian version of the Narcissistic Personality Inventory-16 (NPI-16) as a screening instrument for narcissistic personality. The Indonesian version of the NPI-16 can be used as a valid and reliable instrument to identify narcissistic personality disorder in the Indonesian population. These findings make an important contribution to the development of psychological instruments that are more appropriate to the Indonesian cultural context.en_US
