Adolescents are an age group that is very vulnerable to reproductive health problems, especially STIs (STIs) and unwanted pregnancies (KTD), due to lack of knowledge. This senior practicum report aims to implement community nursing care for adolescents in the Johor Medan Health Center work area in an effort to prevent STIs and unwanted pregnancies through a nursing care process approach starting from Assessment, diagnosis, Intervention, implementation and Evaluation of nursing. The nursing problems that emerged in the five patients were knowledge deficits related to lack of exposure to information. The results of the nursing Evaluation after the implementation of health education showed that the nursing problems of knowledge deficits in the five patients were completely resolved, marked by a significant increase in knowledge in all clients, and an increase in adolescent awareness of STIs and unwanted pregnancies. Providing health education has been proven effective in increasing adolescent knowledge and awareness of the importance of maintaining reproductive health. Continuous education and a peer educator-based approach are recommended to expand the reach of information and increase long-term impacts.
