Immunization is one of the most effective public health strategies for preventing vaccine-preventable diseases, thereby reducing childhood morbidity and mortality. However, full immunization coverage remains below target in Padangsidimpuan City. This cross-sectional study involved 255 mothers of children aged 9–24 months. Data were collected using a structured questionnaire and analyzed through logistic regression to determine factors associated with immunization utilization. Multivariate analysis revealed that maternal knowledge (ExpB = 3.314; 95% CI: 1.927–5.701; p < 0.001) and spousal support (ExpB = 2.208; 95% CI: 1.151–4.237; p = 0.017) were significantly associated with complete immunization. Mothers with good knowledge were 3.3 times more likely, and those receiving spousal support were 2.2 times more likely to complete their child’s immunization schedule. These findings underscore the need for policy interventions to enhance immunization coverage, particularly in resource-limited settings.en_US
