Background: Osteoporosis is a degenerative bone disease characterized by reduced bone mineral density and an increased risk of fractures. It is often accompanied by psychological comorbidities such as anxiety, which may be influenced by biological factors, including uric acid levels and pain severity. Previous studies have demonstrated a complex relationship between uric acid, bone health, chronic pain, and anxiety disorders. Methods: This study used an observational analytical design with a cross sectional approach on 96 subjects . The subjects were osteoporosis patients who met the inclusion and exclusion criteria at Dr. Pirngadi General Hospital, Medan. The Variables measured included uric acid levels (mg/dL), pain severity (Numeric Rating Scale), and anxiety scores (Generalized Anxiety Disorder-7). Data were analyzed using correlation tests to assess the strength and significance of the relationships. Results: This study found that the average uric acid level of 6.51 (r = 0.621, p < 0.005) and the degree of pain (r = 0.361, p < 0.005) were positively correlated with anxiety symptoms. The median age of the subjects was 58.17, with 86.5% of them being women with the highest degree of pain being moderate. The most common anxiety syndrome was moderate. Conclusion: Uric acid levels and pain severity are significantly associated with anxiety symptoms in osteoporosis patients. These findings highlight the importance of a multidisciplinary approach integrating pain management, uric acid control, and psychological interventions to improve patient quality of life.
