    Abstract
    Background: Critical illness is a life-threatening condition that requires intensive care, comprehensive observation, and special care. One of the assessments for early detection of disease severity in critically ill children is using the PELOD-2 score. Previous studies have shown that the Neutrophil-Lymphocyte Ratio (NLR) can be used as a predictor of mortality and the Platelete-Lymphocyte Ratio (PLR) can be used to determine the severity of sepsis. Objective: Analyzing the relationship between NLR and PLR with PELOD-2 score in critically ill children at PICU of Adam Malik Hospital Medan. Method: This Cross-sectional analytical study in critically ill children with inclusion criteria of critically ill children in the pediatric intensive care unit in 2023 and exclusion criteria in the form of children with malignancy, incomplete medical records and autoimmune diseases. Data were taken from secondary data of medical records. Results: 118 subjects, 53.4% was female, 44.9% under 1 year, 68.6% with respiratory disorders. Spearman correlation test analysis showed no significant relationship between NLR and PLR with PELOD 2 score at first day (p=0.983 and p=0.471). There was significant relationship between NLR and PELOD 2 score (p=0.001) on third day. Correlation value (0.311) means increasement of NLR value followed by increase of PELOD 2 score. There were significant correlation between delta PLR and delta PELOD 2 score (p=0.002) with correlation value 0.286, means the increasement of delta PLR value followed by delta PELOD 2 score. Conclusion: There is a relationship between NLR and PLR with PELOD-2 Scores on the third day of treatment. An increase accured in NLR and PLR on the third day which was followed by PELOD-2 scores.
