    Abstract
To analyse the relationship between the neutrophil lymphocyte ratio (NLR) and platelet lymphocyte ratio (PLR) and the severity of dengue infection. Method: This is an analytical observational study with a retrospective cross sectional design aimed at evaluating the relationship between NLR and PLR with the severity of dengue virus infection in patients at RS Adam Malik Medan. Data were obtained from patient medical records from 01 July 2023 to 30 June 2024. The study population consisted of 148 patients, of whom 128 met the inclusion criteria, while 20 were excluded due to hematologic malignancy, autoimmune disease, or coagulation disorders. Total sampling was used to recruit patients diagnosed with dengue virus infection. The variables analyzed included NLR, PLR, and severity classification based on the WHO 1997 criteria. Statistical analysis was performed using ANOVA and Kruskal Wallis tests. Results: This study involved 128 patients admitted to Adam Malik Hospital Medan with IVD. Data showed that 57% of patients were male, and the majority were aged 25 64 years. The examination results showed a median NLR of 1.78 and PLR of 53.4. There was a significant relationship between PLR and dengue severity (p=0.001) but no significant relationship between NLR and dengue severity (p=0.667). Spearman correlation analysis showed a negative correlation between PLR and dengue infection severity (p<0.001, 0.351). The negative correlation indicates that lower PLR values are associated with disease severity. Conclusions: This study showed that PLR value was significantly associated with the severity of dengue infection, the negative correlation means that the lower the PLR value, the more severe the disease severity. In contrast, NLR showed no significant association with severity. Keywords: Dengue Severity, Neutrophil Lymphocyte Ratio (NLR), Platelet Lymphocyte Ratio (PLR), Biomarkers.
